Death of baby girl who was given glucose overdose

This is such a heartbreaking story. Poppy Davies was born 3 months premature and transferred to Great Ormond St for a minor operation. Somehow,(investigations are on going), she was given a glucose overdose. She died on Sunday. How awful for the parents to think that their baby daughter would be receiving the best care and treatment in one of the best children's hospitals in the country, and then for this to happen.

Such a shame, bad enough to have a baby born prematurely, but for her to die like this is just awful

How can this happen, in this day and age? These things are supposed to be checked and RE-checked, especially when dealing with such a tiny fragile human being. I'm so sorry for the parents.
